If your doctor has asked you to submit 7 days of blood pressure readings, please print off and complete the attached form. Once you’ve recorded all your readings, email the form to contact.fernville@nhs.net or bring it back to the surgery.
If you do not have access to your own blood pressure machine, please visit the surgery and use our self-test pod in the waiting room. A member of staff will be happy to help should you need assistance.
